This summary is highly recommended for all applicants.

Compose a list that includes all of your awards and achievements; school activities; employment; school, community, and church leadership; and community service. For each activity, indicate the time commitment of your involvement, e.g., "2 hours a week once a month for two years" or "once for 6 hours."

Focus on your most recent activities, although you may wish to include some highlights from the years you were in high school.

Drawing from the list youve created, make a cover sheet that highlights your most significant activities. Include no more than five activities on this cover sheet. We encourage you to use the space to develop your thoughts about these activities, by recording not just the content and time commitment but also why the activitity was important to you.

Format

Type or print your activities on 8.5 x 11 white paper, and use only one side of each sheet. Print your full name and the date at the top of each sheet.
