What do I need to submit to apply?
Application Checklist for Returning Former UW Students
For applicants who previously have been enrolled in an uncompleted degree program at the UW, a complete application file consists of:
- Returning Student Reenrollment Application.
To apply via the Web or download an application, visit the main UW Web site. When completing the application, it is important that you indicate which major you would like and check the Evening Degree box.
Attention former UW students! The autumn 2008 returning student reenrollment application deadline has been extended to Sept. 1, 2008. However, if you wish to apply after July 1, 2008, you must download the application form and submit it by mail. You cannot apply via the Web for autumn admission after July 1, 2008.
- $50 nonrefundable application fee. *
- Transcripts of any academic work completed after leaving the UW.

Note: Former UW students who have attended the UW only as nonmatriculated, graduate or professional students, or who have completed a degree and are now seeking an additional bachelor's degree must submit the Undergraduate Admission Application for new students along with all required official transcripts to the Office of Undergraduate Admissions. Choose a link in the left column to review admission information and application procedures.
